How to get referred

If you or a family member has been diagnosed with a life-limiting illness like cancer or a neurological condition and your GP is based within our nursing area, we may be able to support you.   The referral process varies somewhat depending on the type of service you require.

Adult Hospice at Home Service

You can request a referral to Rennie Grove's Hospice at Home service from any of the health care professionals involved in your care, including your GP.  Referral guidance for health care professionals can be found on our website here.

Children's Hospice at Home Service

You can request a referral to Rennie Grove's Children's Hospice at Home service from any of the health care professionals involved in your child's care, including your GP.  Referral guidance for health care professionals can be found on our website here.

You may self-refer to this service by simply giving us a call on 01442 890222 or sending an enquiry email to help@renniegrove.org.  If you'd like to progress your referral quickly, you may wish to attach a completed self referral form to your email, which you can download here


Adult Day Services at Grove House

You can request a referral to Grove House Day Services from any of the health care professionals involved in your care, including your GP.  Referral guidance for health care professionals can be found on our website here.

You may self-refer in person by simply calling in to Grove House during one of our regular Drop-in Sessions, when a nurse will be able to meet with you for an initial assessment. Our drop-in hours are Tuesday to Friday 9am-4pm.  Address and directions can be found here.

You may self-refer by phone (01727 731013) or email (help@renniegrove.org).  When self referring by email it is helpful if you can specify your interest is in our Day services and provide a few basic details including your full name, phone number, date of birth, GP surgery, and a brief summary of your reasons for seeking the referral. A clinician will be in touch within 7 days.  Please do specify if you have a preferred method of contact.

 

Family Support Services

If you have a family member or friend under our care, you can request a referral from one of the team members providing their care, or you can contact our family support services team directly as follows:
