Referring to Supportive Care

Referral criteria and process for Rennie Grove Peace’s Supportive Care services.

Referring to Supportive Care

Would your patient benefit from our Supportive Care services?

Our Supportive Care services include counselling, listening, bereavement support, group work and complementary therapy. Designed to support carers and their families in Herts and Bucks, these services can help patients with life-limiting illness too.

For full details of the support this service can provide, please see our Supportive Care service information page.

Our Supportive Care services are for current and past Rennie Grove Peace patients and their family members.

Want to know more about our services before you refer?

Contact our Coordination Centre to find out about our wide variety of over 40 services – for your newly diagnosed patients to those needing end-of-life care. We can help you find the services that best fit your patient’s needs.

We accept both self-referrals and professional referrals for Supportive Care services, as we recognise that people in emotional difficulty may need support in getting help. No formal referral form is required.

Individuals are able to self refer to this service by making an enquiry using the email address or appropriate phone number for their area as given below.

If you are facilitating a referral for a patient, their carer or family member, or an adult in need of bereavement support, please ensure you have obtained their permission before contacting us.
