Would your patient benefit from the Rapid Personalised Care Service?

The Rapid Personalised Care Service (RPCS) offers personalised care in the home for up to 12 weeks for people approaching end-of-life living in Herts or Bucks. The team can make up to four visits a day, 7 days a week, 365 days a year, between the hours of 7am and 11pm. By providing help with things like meal preparation, washing, dressing, and toileting, this service gives terminally ill people the daily support they need to stay safe and comfortable at home during their final weeks.

  1. Patient has a diagnosis entering the terminal stages.
  2. Patient has a poor prognosis likely to be less than 12 weeks, they have an irreversible condition that no longer responds to medical treatment/ intervention, DNAR in place.
  3. Patient is rapidly deteriorating and requires urgent care services.
  4. Patient must be registered with a Herts Valleys or Bucks GP.

To refer a Hertfordshire patient for our Rapid Personalised Care Service, please:

  1. download the RPCS referral form for Herts
  2. download the RPCS consent form for Herts
  3. complete both forms and submit to westherts.rpcs@nhs.net.

Referrals must be made by a healthcare professional involved in the patient’s care.

Please complete a CHC FastTrack and consent, stating ‘RPCS appropriate’ and send as normal to:

This is a twelve week care package. Should the patient stabilise or require continuing care beyond twelve weeks, an assessment will need to be made and provision of care transferred to another service.
